Here are some pictures we took after a cylinder rupture.

On your left shows 2nd cylinder valve severed at cylinder neck. To the right of that is the bleeder valve. On the far right is 3rd line valve from unused fill whip. External dents in fill station. Cylinder valve O Ring stretched over valve housing-No thread damage. Notice how 2nd valve is sheered and cylinder is dented. On the right you can see how the rupture drove the cylinder into the rear of the fill station denting the 1/4 steel housing.
